Information about the medical exam for US
Green Card applicants
We provide the cheapest check up for
the US Green Card in the Czech Republic.
Due to our long association and cooperation with the American embassy, our fee for this exam is currently the lowest in Prague. The fee for the exam is 3200 CZK (not including blood tests ) for adults as well as for children. All payments for exam, tests and vaccines if applicable, can be done by cash or credit card at our reception desk.
The following is information regarding procedure and needed tests
HIV and Syphilis blood testsHIV (AIDS) and Syphilis blood tests are required for the exam. These blood tests must be performed only at our lab at the Polyclinic and test results will be ready in 30 minutes. Children under 16 do not need these tests. The total cost of both tests is 800 CZK.
Chest X-RayAn original chest X-ray, less than 3 months old, with your name and date on the X-ray, is required. Reports of X-rays without the X-Ray are NOT accepted. Children under 16 and pregnant women (report of pregnancy needed) do not need an X-ray. If you do not have an X-ray, we shall be happy to arrange one for you a short stop away, the same day that you are here.
Valid passportA valid passport is required for presentation at the exam. If a minor does not have a passport, they must be registered in the passport of parents.
PhotosTwo passport photos 3X4cm format are required for the exam.
Physician summaryA short summary from your personal physician outlining any significant medical history of illness, medications, as well as current medical status is required for the exam.
Immunization recordAll persons born after 1960 need to present a complete immunization record for review, including all routine childhood immunizations and history of childhood illnesses. This includes, but is not limited, to the following:
* tetanus, diphtheria, pertussis (DPT)
* polio
* measles (morbilli)
* mumps (parotitis)
* rubella (rubeola)
* chicken pox (varicella)
All persons under 18 years old must also be vaccinated against Hepatitis B. This is a series of three vaccines. It is enough that the first vaccine of the series be given by our physicians to fulfill the requirement.
All children 2 months old to 5 years old must also be vaccinated against Haemophilus B.
For persons born before 1960, the only vaccination requirement is evidence of tetanus booster within the past 10 years.
All persons older than 65 must also have evidence of vaccination against pneumonia (Pneumovax vaccine).
Our physicians can provide all of the above vaccinations in the event that you need to be brought up-to-date. We can also provide laboratory evidence of previous immunity to any of the above illnesses.
